TRAUMA TORACICO

The first division between continents was made by ancient Greek navigators, who named them 'landmass, terra firma'.

Generally classified by convention rather than any strict criteria, nowadays seven regions are regarded as continents from a geopolitical point of view.

ROTURA DIAFRACMATICA

-Administracion de oxigeno suplementario a concentraciones altas -Dar soporte ventilatorio

-Se debe tratar de llevar al paciente lo mas rápido posible al centro de salud -Evitar peritonitis con el transporte rápido

-Dificultad respiratoria aguda -Ansioso -Taquipneico -Pálido -Crepitaciones ósea -Enfisema subcutáneo -Ruidos hidroaéreos en el tórax -Disminución del murmullo vesicular en el lado afectado -Forma escafoidea del abdomen

ROTURA TRAQUEOBRONQUIAL

-Administracion de oxigeno suplementario -Asistencia ventilatoria -Esta contraindicado la intubación selectiva en la prehospitalaria ya que puede empeorar la lesión bronquial

-Es imperativo el monitoreo continuo en cuanto a signos de progresión hacia el neumotórax a tensión, y debe intentarse la descompresión rápida con aguja si se presentan esos signos.

-Dificultad respiratoria -Pálido -Diaforético -Uso de músculos accesorios -Gruñido -Aleteo nasal -Enfisema subcutáneo extenso en la parte alta del tórax y el cuello -Distención de las venas yugulares -Frecuencia ventilatoria elevada y SO2 disminuida -Hipotensión

CONMOCION CARDIACA

-Se inicia RCP con desfibrilación eléctrica -Si los intentos inmediatos de desfibrilación no tienen éxito, se asegura la vía aérea y se inicia un acceso IV. -Administramos epinefrina y fármacos antiarrítmicos

-La conmoción cardíaca se trata de manera similar a los paros cardíacos resultantes de un infarto de miocardio -Debe determinarse el ritmo cardíaco tan pronto como sea posible con aplicación rápida de desfibrilación si se identifica fibrilación ventricular

-Paro cardiorrespiratorio -Equimosis sobre el esternón -Fibrilación Ventricular -Bloqueo cardiaco completo y bloqueo de rama izquierda del haz de hiz con elevación del segmento ST.

TAPONAMIENTO CARDIACO

- Se realizara una Pericardiocentesis con aguja para drenar el liquido almacenado. - Pocas veces se realiza una toracotomía de reanimación

MANEJO O TRATAMIENTO

- Se debe trasladar de manera urgente y lo mas rápido posible a un centro de salud donde pueda realizarse la reparación quirúrgica. - se ayuda al paciente con oxigeno -Se realiza una canalización de vías para dar tratamiento con líquidos. - Con la fluidoterapia se mantendrá la presión venosa central elevada y mejora el llenado cardiaco.

-Hipotensión y reduce el gasto cardiaco - Herida abierta por arma de fuego o arma blanca. -Pulso paradójico -Presenta la famosa triada de Beck; la cual sugiere un taponamiento cardiaco: 1. Tonos cardiacos atenuados o distantes. 2. Distención de la vena yugular. 3. Hipotensión arterial.

CONTUSION PULMONAR

-Se debera poner toda la atencion en la ventilacion del paciente. - Se deve administrar oxigeno en todo paciente que se sospeche una contusion pulmonar -Se debera reevaluar de forma repetitiva la frecuencia respiratoria y cualquier signos de dificultad respiratoria.

-Hemorragia en los espacios alveolares -Presenta estertores al auscultar y frecuencia aumentada según avanza la contusión. - Puede haber dificultad respiratoria con el avance de la contusión.

-Plétora o estado caracterizado por un exceso de sangre. -Edema -Distención de los vasos sanguíneos - Eritema subcutáneo -Cianosis en cara, cuello, y parte superior del tórax.

ROTURA TRAUMATICA DE LA AORTA

-Oxigeno suplementario a altas concentraciones -Acceso IV - Es factible controlar tanto la presión arterial como la fuerza contráctil cardiaca con la administración de bloqueadores B. -se monitorea la presión arterial y se mantiene en su rango normal mediante soluciones o medicamentos que permitan este proceso como son lo bloqueadores B

Se comunica en primera instancia al centro medica acerca del mecanismo y la sospecha de rotura aortica. -Es importante no elevar la presión arterial de manera intensiva, ya que podría llevar a hemorragia exanguinante y así a un shock.

-Diferencia de pulsos en ambos brazos -Diferencia de pulsos arteria humeral y femoral

LESION POR CONTUSION CARDIACA

-Oxigeno a concentraciones altas -Se establece un acceso IV para la administración de soluciones -Si existe arritmias, debe instituirse la terapia eléctrica o farmacológica.

-Se debe evaluar minuciosamente si existe la contusión miocárdica. -Se coloca al paciente en un sistema de monitoreo cardiaco para asi detectar arritmias y elevaciones del segmento ST.

-Taquicardia sinusal -Shock cardiogénico -Equimosis sobre el esternón -Disnea y Dolor torácico -Crepitaciones sobre el tórax y su inestabilidad -Costillas rotas acompañadas de movimientos paradójicos
